I. OHIP covers visits, the procedure and lab tests.

II. We have a no show and late cancellation fee of $300.00.  48 hours cancellation notice is required. 
To secure appointments with us we take a deposit of $50.00 by credit card at the time of booking.  This will be applied toward any fees listed below.

III. We charge a fee of $75.00 for a block of services, and $50.00 for supplies that are not insured by OHIP for a total of $125.00. 
Government regulations require that these costs are optional. Uninsured services and supplies can be paid for individually or not purchased at all.
We remind patients that these are a central part of the convenient service The Gentle Vasectomy Clinic provides and that declining to accept some of them will likely lead to significant inconvenience.

Uninsured services are covered by the $75.00 Block Fee. 
Costs for these services purchased separately are noted here:
-reporting semen analysis lab results by email or phone -  $50 for each test (sometimes more than one test is needed until no sperm are present).
-time talking with doctor on phone about any problems after the vasectomy -  $50 for each call
-filling work forms and notes -  $25 each
-mailing or faxing further lab requisition forms -  $25 each
-phoning or faxing prescriptions to a pharmacy - $25.00 for each call/fax
(Note that all of these together total $175, covered by the $75.00 Block Fee).
