I. OHIP covers visits, the procedure and lab tests.

II. We charge a Block Fee of $150.00 for supplies and services.  This is paid by credit card at the time of booking the appointment over the phone.  It also serves to secure the appointment. 
We remind patients that these are a central part of the convenient service provided by
The Gentle Vasectomy Clinic .

Supplies and services covered by this are: 
-kit of supplies for care before and after the vasectomy.    This includes the athletic support;
-reporting semen analysis lab results by email or phone;
-time talking with doctor on phone about any problems after the vasectomy;
-completion of  work forms and notes;
-mailing or faxing further lab requisition forms;
-phoning or faxing prescriptions to a pharmacy.

A statement with details of our supplies and services will be provided upon arrival at the Clinic.

III. We have a no show and late cancellation fee of $300.00.  We require 2 full business days notice of any cancellations.
