I. OHIP covers visits, the procedure and lab tests.

II. We have a Block Fee for supplies and uninsured services.  This Fee is $200.00 plus 13% HST.  This is paid by credit card at the time of booking the appointment over the phone.  This fee is also to secure the appointment.
We remind patients that these are a central part of the convenient service provided by The Gentle Vasectomy Clinic .

Supplies and services covered by this are:  
-kit of supplies for care before and after the vasectomy.    This includes an athletic support, ice pack, ointment, gauze, EMLA patch, Ativan tranquilizer, Advil, acetaminophen, a container for a semen sample;
-reporting semen analysis lab results by email or phone;
-time talking with doctor on the phone or email about any   problems after the vasectomy;
-completion of  work forms and notes;
-mailing or faxing further lab requisition forms;
-phoning or faxing prescriptions to a pharmacy.

A statement with details of our supplies and services will be provided upon arrival at the Clinic.

III. We have a no show and late cancellation fee of $300.00 + HST for a total of $339.00.  We require 24 hours notice of any cancellations.
